Home Health
We provide skilled nursing care in the comfort of home for clients who require medical support and ongoing monitoring.
Services begin with an assessment by a Registered Nurse (RN), who develops a care plan based on each client’s individual needs and provider referral.
Home Health services may include:

Specialty wound care

Medication administration and management
-
Intravenous (IV) medication therapy

Lab and blood specimen collection
-
Ongoing health monitoring and assessments
A provider referral is required for Home Health services.
Home Care
We provide hands-on support to help clients remain safe, comfortable, and independent at home.
Our caregivers and Certified Nursing Assistants (CNAs) or Nursing Assistants (NAs) assist with daily activities and personal care based on each client’s individual needs.
These services may include:
Activities of Daily Living:

Bathing and personal hygiene

Dressing and grooming

Mobility and transfers
-
Toileting and incontinence care

Eating and feeding assistance
Instrumental Activities of Daily Living:

Meal preparation
-
Light housekeeping
-
Laundry
-
Medication reminders

Transportation to appointments or errands
A provider referral is required for Home Care services.

We welcome visiting healthcare providers and specialists who offer additional services here on Lānaʻi, helping make care more accessible for our community close to home.
These providers offer specialty care and services throughout the year by appointment. Referral requirements may vary by provider.
