Home & Community Services
Serve clients in their home environments or at a community location, as preferred by the individual or the family members.
Home Care
Under the supervision of the Registered Nurse (RN), the trained caregivers, typically Certified Nursing Assistants (CNAs) provide support with basic activities & functions referred to as Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).
Home Health
Client services which require skilled nursing support. Typically, a Registered Nurse (RN) assessment, and implementation of a plan of care, to include interventions based on the primary care provider (PCP) orders. Such as, specialty wound care, administration and management of medication(s) in the client’s home environment, including but not limited to intravenous (IV) medications administration, lab/blood collection, health monitoring, etc. as indicated and ordered.
